What information this website collects
The site includes no form-submission backend. The optional community form validates information in the browser only and, after explicit confirmation, can open a draft in the visitor’s own email application. The site code does not save the entered name, email address, selected interest, contact messages, mental health details, addiction information, or crisis disclosures.
Form values are cleared when the page closes or reloads. No form responses are stored in localStorage, cookies, IndexedDB, analytics, or application databases.
Purpose of collection and consent
This static site does not perform form submission or application-level collection. Before any future public form collects information, the organization must identify a clear purpose, collect only the minimum needed, and present meaningful consent at or before collection. Newsletter or community consent must remain separate, specific, and optional, with a clear way to unsubscribe.
Use and disclosure
This static website does not receive form submissions. Information included in an email draft is controlled by the visitor and their email provider once they choose to send it. Visitors should not send urgent, crisis, or highly sensitive personal health information through ordinary email.
Safeguards and retention
The static site restricts network connections through its Content Security Policy and does not include trackers or remote dependencies. It does not retain form values. Any hosting provider may maintain ordinary request logs under its own configuration and policies. A public hosted version requires documented administrative, technical, and physical safeguards, approved retention schedules, access controls, incident response, and vendor oversight.
Access and correction
Because this site code keeps no form records, it has no stored form information to access or correct. For questions about information held by the organization or hosting provider, contact FNP using the privacy contact below. The responsible organization must confirm the formal privacy process before public launch.
Third-party services, cookies, and analytics
The static build contains no analytics, advertising, fingerprinting, embedded social feeds, non-essential cookies, remote scripts, or third-party form and donation processors. External links lead to third-party websites with their own privacy practices.
Any future analytics, newsletter, donation, hosting, or form integration requires an updated privacy assessment, contractual review, consent design, and changes to this notice.
Children and youth
The service supports youth, but this website does not knowingly collect or store information about children or youth through forms. Any future collection involving youth requires age-appropriate notices, consent and capacity review, strict data minimization, safeguarding, and approval by Sunnybrook and qualified legal and privacy personnel.
Changes and privacy questions
This review-ready notice must be updated when website practices change. Material changes should be communicated clearly, with the effective date recorded before publication.
